Staff and technical facilities

HC Ljubljana has 1,400 employees, more than 430,000 patients and more than 2.5 million clinic visits, which makes it the largest public primary healthcare institution in Slovenia.

Within the framework of the Municipality of Ljubljana (MOL), HC Ljubljana offers its services and its activities not only to the people of MOL, but also to others seeking medical attention. Thus HC Ljubljana caters to a much larger number of users than the population living in the area of MOL, as it also includes commuters.

HC Ljubljana’s activities aim at providing comprehensive medical care to patients in the fields of prevention, treatment and rehabilitation.

Ljubljana Healthcare Centre is a learning institution, which means it was awarded by the Ministry of Health the title of a Learning Institution to carry out practical training and education for the following programs:

• secondary specialized educational program for nursing technician and laboratory technician,

• secondary vocational educational program, nurse-caregiver, higher professional educational program in nursing, radiology, physiotherapy, occupational therapy and laboratory biomedicine, and

• university program in medicine, dental medicine, and special and rehabilitation pedagogy. Moreover, HC Ljubljana is an educational institution also due to the fact that it continuously organizes internal training for its employees and education for other professional and lay public (CPR techniques).

We regularly cooperate with external institutions such as the Association of Slovenian Healthcare Institutions, Health Insurance Institute, medical schools, medical faculties and schools of the University Medical Centre...

We are constantly introducing innovations such as: Reference clinics of family medicine, regular annual assessment interviews, personal training plan, the new simulation center...

Almost three years after the introduction of Model Practices (MPs), the Ljubljana Healthcare Centre has 43 MPs from the current 353. Throughout the duration of this continuous Model Practice Project, which is constantly updated and upgraded, the project manager takes into account the experience gained in Ljubljana Health Centre MPs (suitably manageable and controllable group) which is transferred and used for the needs of all currently enrolled 353 MPs throughout Slovenia. In this respect, the upgrading of MPs within HC Ljubljana with additional members of the team (home care nurse) is a controlled introduction of innovations into manageable groups of MPs, which through a family approach also raises awareness on content on obesity as a risk factor. On the basis of the ongoing steps of quality check done by the DNV (Norwegian partner), HC Ljubljana and its partners will prepare written documents / protocols and training for professional staff as well as workshops for the general public with the content of lifestyle improvement.

The content intended for the professional staff will be focused on prevention and planned monitoring of chronic diseases, particularly COPD, asthma, diabetes and hypertension, and obesity in the family as one of the major risk factors for chronic diseases. At the same time, education for the general public will take place, during which colleagues from HC Ljubljana will present the importance of a healthy lifestyle to the general public.

The CPC+ project in HC Ljubljana will altogether educate at least 150 professionals and at least 150 individual members of the general public. Lessons and useful content will be transferred onto the national level, HC Ljubljana will therefore present this project as a model of introducing the planned upgrades in MPS into the entire Slovenian primary healthcare - Family Medicine. Ljubljana Healthcare Centre participates in all project work packages, in particular, it will supervise, coordinate and guide the project in its integrity, and will be responsible for its success. Results of the upgraded MPs will be used at the national level for all existing and new MPs. In an outreach to the general population, approaches / measures used by HCL for the residents of the Municipality of Ljubljana (MOL) will also be presented in a broader setting (European Healthy Cities) and in all MPs throughout Slovenia.

University of Ljubljana, Faculty of Medicine

Faculty of Medicine is a member of the University of Ljubljana. It was founded in 1919 and it currently employs 852 workers, of whom 236 are professors.

Faculty is represented by the Dean Prof. Dr Dušan Šuput, with the faculty leadership: Vice-dean for Professional medical field Prof. Dr Srečko Koren, Deputy dean for Scientific research area Prof. Dr Tadej Battelino, Deputy dean for Academic field of Dental medicine assoc. Prof. Dr Igor Kopač, Associate Dean for the Academic field of medicine Prof. Dr Igor Švab and Secretary of the Faculty Nastja Babšek Omahen.

In organizational terms, the faculty is divided into the Department of undergraduate study program in medicine, led by the Vice-dean for Academic field of medicine Prof. Dr Igor Švab, Department of undergraduate study program of Dental Medicine, led by the Vice-dean for Academic field of dental medicine assoc. Prof. Dr Igor Kopač, Department for Professional medical activity, led by the Vice-dean for Professional medical field Prof. Dr Srečko Koren and the Department of Postgraduate Studies, headed by Prof. Dr Vito Starc. Teaching and research work is carried out in the departments and institutes, and individual centers and labs within a single faculty institute.

Department of Family Medicine at the Faculty of Medicine, University of Ljubljana was established by a decision of the Chamber on 27 November 1995. In a short time we managed to involve many young and ambitious colleagues, who wanted to pursue an academic career. Later, the rapid increase in the number of assistants, as well as new PhDs and teachers followed. We began our cooperation with experts also from other disciplines.

The Department of Family Medicine at the Faculty of Medicine, University of Ljubljana (Partner 2) will participate in virtually all work packages of the project, since it represents the highest academic authority on the proposed project on the national, i.e. Slovenian level and is therefore a natural holder institution of WP 3 and one of the main creators of the educational model of the CPC + project, since its wide circle of associates not only extends to but also complements experts from other academic backgrounds. The primary task of the Department of Family Medicine in the project is to lay the theoretical foundations (WP 3) and to, in collaboration with other partners, educate professionals (Work Package 4) for the implementation of virtually all education of instructors. Due to its central academic roles, the Department is at the same time involved in all stages of dissemination and evaluation project (Work Package 2 and 6).

The Municipality of Ljubljana (MOL)

Ljubljana is the political, scientific and cultural center of Slovenia, with more than 280,000 inhabitants it is the largest city in Slovenia and its most important economic centre. It consists of 17 urban communities that allow for an even closer cooperation with local residents. Despite the fact that it is classified among medium-sized European cities, it maintains the friendliness of a small town and at the same time possesses all the characteristics of a metropolis, covering an area of 274.99 km2. The Municipality of Ljubljana has been a member of the European Network of Healthy Cities, which operates within the World Health Organisation for the past 25 years.

MOL is the most important contact with the population of central Slovenia and is therefore the natural institution of the role of publicity and direct contact with citizens (WP 2), which have not yet been diagnosed and for them it is even more important and reasonable to prevent lifestyle-caused diseases. The Municipality of Ljubljana will also participate in all other work packages with the resources they have available and can contribute to a more efficient implementation of the project. Especially suitable is its, on one hand, central location in the Republic of Slovenia, but on the other hand also the division on highly rural areas where the activities of the project realization(Work Package 5) can reach the population that rarely decides to visit the healthcare centre or take part in informative workshops on healthy lifestyle.

The Chamber of Nurses’ and Midwives’ Associations of Slovenia - Association of professional nurses, midwives and nursing technicians of Slovenia

The Chamber of Nurses’ and Midwives’ Association of Slovenia - Association of professional associations of nurses, midwives and nursing technicians of Slovenia is a professional, non-governmental and non-profit association in Slovenia, which brings together more than 15,000 nurses, midwives, medical technicians and paramedic carers with its long tradition of professional activity. A uniform and professional representative body for providers of Nursing and Midwifery care in Slovenia, professionally operates in 31 technical sessions that examines technical issues, organizes professional training and develops professional recommendations and guidance from their focused field of expertise. Only within the technical sections, about 5,000 performers of Nursing and Midwifery care are being educated annually. The Chamber - Association consists of 11 regional professional associations, enabling its professional regional operations countrywide. It is a member of the International Council of Nurses (ICN), International Confederation of Midwives (ICM) and the European Federation of Nurses Associations (EFN).

The Chamber - Association (partner 4) is a soundly-organized professional organization, with its 15,000 members operating throughout Slovenia. Its role in the work package 3, 4 and 5 is indispensable because it is not only actively involved in the layout design of the CPC +, education and realization, it can provide the notifying and informing both the Slovenian and international public through its rich links (WP 2).

Institute Prava poteza

Prava poteza is a research and development institution established in 2005 and is registered with the district court in Maribor, Slovenia. The Institute’s processes are ICT-supported and tailored to project management and as an advisory and educational institution it is situated in Vrhov Dol in Limbuš with abusiness unit on Kraigherjeva street 5, Maribor.

Institute of Prava poteza is engaged in providing advice on the application and implementation of projects, including grant funding from the European Union and the Republic of Slovenia. Seasoned in a multi-project management, they are ready to face all the situations that occur in the ever dynamic project environment.

Subsidy applications demand, alongside very clearly specified objective and strategy, also a carefully executed administrative implementation of the application itself. They are pleased to offer assistance in administrative implementation and control of EU (also in English) and local tenders. Prava poteza has in all the years of its continuous operation ensured a coordinated technical administrative support in the areas of business competence, business development, e-learning, e-health and general management. They work with a network of highly creative and technically skilled professionals in scientific, business, public and private organizations. Their professional and flexible knowledge based on decades of experience in managing technology-supported by ICT projects in different contexts and levels. From the 18 international projects (references are listed on the website), financed by EU funds, in which we have been involved as partners or outsourcers, its role in this report is most closely related to: Evaluation of the project ACTrain, dissemination project DRA-MUR-CI and management of the project office for Model Practices at the Ministry of Health (2013), in close liaison with the professional and the design council of MPs, in which all the most influential associates and partners of the project CPC + were included.

The role of Prava poteza is essential (Partner 5) in workpackage 1, WP 2 and WP 6, where they provide the project with its experience and knowledge of ongoing work, supervision and support in the implementation of the substantive work packages of the project.
