The project "Upgraded Comprehensive Patient Care (CPC +)" has been carried out since March 2015, its main objective being reduction or prevention of lifestyle-related diseases. The City Hall held a ceremonial signing of the partners on April 17th 2015.

The contract was signed by:
Rudi Dolšak – Ljubljana Healthcare Centre, as the CPC + developer, whose main purpose is the integration of community nurses, implementation of education of patients on risk factors with the help of simulations in the SIM center, and family treatment of obesity.
Krešimir Antonio Paliska - a DNV partner representative, whose main role is to ensure quality in all stages of the project.
Prof. Dr Igor Kopač - a representative of the partner Faculty of Medicine, University of Ljubljana, whose primary mission is to educate specialists in family medicine.
the Mayor Zoran Janković - Municipality of Ljubljana, which as a partner informs the public about the project and its content.
Andrej Vojnovič – Nurses’ and Midwives’ Association of Slovenia - Chamber of professional associations of nurses, midwives and nursing technicians of Slovenia - its role is to participate in model CPC + setup and to facilitate the realization of the education of community nurses.
Tanja Kocjan Stjepanovič - representative of the partner Institute Prava poteza with its project experience and expertise ensures ongoing work, supervision and support in implementing the project.

Doc. Dr Tonka Poplas Susič, MD spec., Medical Director of HC Ljubljana, stressed at the event that: "This partnership will provide a nationwide pathway for primary healthcare, that is based on the experience of the Healthcare Centre Ljubljana."
