One of the aims of the project CPC+ project was to educate 50 community nurses to enable a new way of working in the field. For this purpose, we carried out seven sets of trainings that were completed on September 30th, 2015 with the last module being Family Treatment of Obesity.

Community nurses have acquired new knowledge in the fields of preventive patient care; treatment of chronic patients with asthma, chronic obstructive disease, hypertension and diabetes mellitus type 2 in the home environment/outside of practice. Protocols for nurses in Model Practices of Family Medicine, which are adapted to work in the field, were used as a basis for all the aforementioned training. With this, we wanted to standardize the treatment of patients in home environment/in the field by community nurses with the treatment provided by the nurses in the MPs, thus allowing for the same quality treatment to those patients who cannot acces primary care clinics.

In addition to these trainings, community nurses were presented with a module on Effects of Family on Health and providing a safe environment for the elderly, and other topics essential to a quality patient treatment outside the clinic. Moreover, within the Family approach to Obesity treatment module, a new perspective on obesity treatment and identification of malnutrition levels using MUST toolkit was presented.


One of the aims of the project is to strengthen the multidisciplinary cooperation in patient care, especially the cooperation between the community nurse and the MP team. To this end, the module MP team Training was carried out, where in addition to the Family Treatment of Obesity module, we presented the work of community nurses, methods of cooperation and the role of the SIM Centre. For the purposes of patient care in the home environment, we equipped community nurses with the necessary documentation:

  • Community Nurse Report which serves the purpose of reporting of reporting of the community nurses to the MP team, referrals from RADM to PV and referrals of patients to the SIM Centre; - Questionnaire for preventive examination in the area of ​​chronic noncommunicable diseases in the field;
  • Morse Fall Scale;
  • Family APGAR questionnaire;
  • ACT questionnaire;
  • COPD Assessment Test (CAT) questionnaire;
  • Scale of Pain Intensity by VAS scale;
  • SIM Centre pamphlet;
  • Pocket guide to evaluating risk of malnutrition;
  • Oral nutritional supplements and nasogastric intubation;
  • Practical Guide for general practitioners and other health professionals;
  • Assessment of readiness for change.

Community nurses began with new ways of working in the field in the beginning of October.
We believe that new knowledge and new approaches to the patient treatment by the community nurses will contribute to higher quality health care for patients in their home environment.
